             I freely, voluntarily, and in my own interest, give my consent for the private institution of higher education Saint Tikhon’s Orthodox University of the Humanities (herein: STOUH), official (postal) address: Ul. Novokuznetskaia 23B, Moscow, 115184, to process (understood to mean any action/operation or group of actions/operations performed on personal information with or without automatic means, including collection, recording, systematization, accumulation, saving, correction [updating, modification], extraction, use, transmission [distribution, provision, access], anonymization, blocking, deletion, destruction) the following personal information:
      - surname and given name(s);
      - date and place of birth;
      - citizenship;
      - officially registered and actual address;
      - date of registration at place of residence;
      - photograph;
      - knowledge of foreign languages and languages of the Russian Federation;
      - information about academic performance, indicators about the educational process, participation in the University’s scientific and educational work, rights to intellectual property created during the period of study at the University (as a part of the educational process and scientific work);
      - information on disciplinary action;
      - bank details;
      - information about education;
      - phone number and e-mail;
      - other personal information whose processing is provided for by the legislation of the Russian Federation and which is necessary for studying at the university.

              I provide the personal information listed above for processing for the purpose of ensuring that the legislation of the Russian Federation regulating relations in the area of education and those closely related to the same, is observed and carried out with respect to my person, that is, for purposes of carrying out obligations imposed by legislation on education, for purposes of concluding and fulfilling contracts on education; accounting and taxation, record-keeping, registration, and participation in the educational process through the Distance Learning System.

             I have been informed that:
      1) my consent to the processing of personal data is valid from the date of signing the present agreement for the entire period of study at STOUH;
      2) my consent to the processing of personal data may be withdrawn on the basis of a written application in form;
      3) in the event of withdrawal of the consent to processing of personal data, STOUH is entitled to continue processing personal data without consent of the student if the grounds specified in Paragraphs 2–11 of Article 6.1, Article 10.1, and in Article 10.2 of the Federal Law “On Personal Information” are met;
      4) after exmatriculation from STOUH (termination of educational relations), personal information is retained by STOUH for 75 years, a long as another period of time for document storage is not stipulated by currently effective legislation of the Russian Federation;
      5) personal data provided by students in relation to third parties will be processed only for the purpose of implementation and performance of the functions, powers, and duties imposed on STOUH by the legislation of the Russian Federation.

            In signing the present consent form, I furthermore confirm that I have been acquainted with the Regulation on Personal Data confirmed by Decree No. 1-061/01 of the Rector of STOUH on 24.02.2012 (as amended by Decree No. 1-863/01 of 22.10.2018).
